footswitch with banana jacks and phono plug

I use a regular ol' lab power supply.
so the footswitch leads have the banana jacks to the power supply and then a 1/4 phono jack plug down toward the actual footswitch.

So the phono end of the rca cord would plug into the phono jack located down by the footswitch.

I found this set up a while back and can't kwite put my finger on where i got it.
I hope i don't sound crazy the way i explained it.

does anyone know where i can find these magical footswitch combos???
I really don't have any other choice because of the power supply I am using.

Why not show us some pics? And yes you can't go wrong with Thom fixing you up,not only will it be totally reliable but you can bet he'll come up with something that really looks the b*llocks.
